Whose name and address appear above on this document, agrees to pay the Service Provider, Superlative Systems Integration, Inc. A $75 dollars diagnostics fee Per Computer. Plus, applicable taxes. Superlative Systems Integration, Inc. will then perform a diagnostic of your Computer and generate a report of findings that will need to be corrected. We will then furnish an estimate with parts, labor and software that shall be needed to remediate your computer’s issues. If the estimate is accepted by you the End User within 30 days of the diagnostics report and you agree to go ahead with the repair, we will then discount your Diagnostic fees from your service invoice. However, if you decide not to pursue the repair of your computer, your diagnostics fee is non-refundable. I will also allow Superlative Systems Integration, Inc. 72 hours or 3 business days to perform a full diagnostic. I the End User also understand that I may or may not lose data during this process or during the process of repair and will not hold Superlative Systems Integration, Inc. accountable or liable for any such loss of data as a result of this repair. I understand that I am (End User) responsible for backing up any such data files on a separate storage device. I have read and understand this agreement. I the End User have made no other agreement with Superlative Systems Integration, Inc. verbal or written. Repair: I agree to give Superlative Systems Integration, Inc. The Authority to upgrade, repair, swap parts, install software and make the changes necessary to my Personal Computer, for the sole purpose of repairing and resolving the issues found in the diagnostics report. I will also allow Superlative Systems Integration, Inc, 15 business days to make such repairs from the date of this agreement. I the End User also understand that I may or may not lose data during this process or during the process of repair and will not hold Superlative Systems Integration, Inc. accountable or liable for any such loss of data as a result of this repair or upgrade. I understand that I (The End User) am responsible for backing up any such data files on a separate storage device. I have read and understand this agreement. I the End User have made no other agreement with Superlative Systems Integration, Inc. verbal or written. Superlative Systems Integration, Inc. Warranties: All Parts are covered by manufactures Warranty. All Bench diagnostics and Repairs shall have no guarantees, written or orally. Due to the nature of a Diagnostics or repair, the Service Provider cannot warranty any third-party device, equipment, electronic components, software, hardware or communications and Electrical supply to any third party manufactured device in which the service provider as no guarantees from these parties. All Labor rendered is nonrefundable. The main purpose of a Service Request is to identify the cause of all issues. Once the issues are identified, additional parts or Products may be required along with a follow up Service Ticket to attempt to repair. All products purchased from us are covered by a manufactures warranty. Defective products shall be replaced by the same model number. Additional Service hours will also have to be purchased to cover the additional time. Additional Service hours can be purchased at an additional cost. All services, repairs and installations shall be performed between the hours of 9am to 5pm weekdays, exclusive of Saturdays, Sundays, and legal holidays. It may take up to 72 hours for a Service technician to respond to a service call. This contract is for sales and services only and is taxable, please provide us a copy of your tax-exempt certificate for our records. All products are covered by a manufacturer’s warranty.
